Medical history form

To save time at your appointment please download the medical history form, complete it and bring it to the practice. You can discuss any points or queries at your dental appointment.

Emergencies

During surgery hours please telephone the practice and we will make every effort to see you as soon as possible.

For out of hours emergencies please contact 111 for help and advice.

Accessibility

The practice is situated on the first floor accessible by stairs only. We have made the stairs as manageable as possible and are happy to provide assistance if required.

Reminders and recalls

Regular routine appointments are recommended. At the end of your course of dental treatment we will arrange your next appointment or set a recall for a text to be sent nearer the time. Text reminders are sent to all patients.

Comments and complaints

We hope that you are entirely satisfied with the dental care and service you receive at Tudor House. We welcome all feedback and use it to improve our services. If you have any comments you would like to make, please either discuss this with a member of staff or put your comment in our suggestions box located in the waiting room. Alternatively please contact our practice manager, Alison who is responsible for our complaints procedure.
